This is offical notice that EDS will no longer be accepting faxed copies of enrollment information due to the extention of the credentialing date from May 1 to August 1.  Please send all information to EDS:

Here is the easiest way to find the Provider Update Form.  You need to fill out Box 1 and Box 20 then any other box that you are wanting to update.
